NOTICE: April 3, 2023

The Illinois Department of Public Health has been granted authority by the Nursing Home Care Act to ensure that nursing homes in the state are in compliance with the act. The Department has determined that on October 15, 2022, Fairview Haven was out of compliance with the act, because we were short .38 hours of RN hours (don't miss the decimal point). 

On average during the fourth quarter of 2022, Fairview Haven was OVER the minimum staffing requirements by 23.24 RN hours (almost double the minimum staffing requirements).
Also - on that particular day (Oct. 15, 2022) - Fairview Haven was well OVER the minimum staffing requirements for total nursing hours.

The Illinois Department of Public Health is being very gracious to waive the imposition of the $16.86 penalty for this violation of the act. However, in the future, should Fairview Haven not be able to maintain adequate hours of RN staffing, we will be penalized.

The Illinois Department of Public Health has required Fairview Haven to submit a plan of correction as to how we intend to fix being short .38 RN hours on Oct. 15, 2022. In the meantime, it is necessary that we post this notice for the public to know of this violation.
